You can access our service if you are aged over 18, live in Pembrokeshire County Borough and you have:

  • a learning disability
  • physical disability
  • a sensory impairment
  • you are a carer
  • you are a vulnerable adult due to age
  • health condition or assessed need  (including autism, ASD, dementia, mental health issues and acquired brain injury)

You must also be in receipt of social care services in Pembrokeshire and/or are eligible to receive a service under the Fair Access to Care (FACS) criteria that prioritises eligibility for care and support within the NHS and Community Care Act.
