You can usually access our service free of charge if you are aged over 18, live in Pembrokeshire County Borough and you have a learning disability, physical disability, a sensory impairment, you are a carer, you are a vulnerable adult due to age, health condition or assessed need, including autism, ASD, dementia, mental health issues and acquired brain injury. You must also be in receipt of social care services in Pembrokeshire and/or are eligible to receive a service under the Fair Access to Care (FACS) criteria that prioritises eligibility for care and support within the NHS and Community Care Act.
