You can access our service if you are aged over 18, live in Powys County Borough and you either:

• have a learning disability
• have a physical disability
• have a sensory impairment
• are a vulnerable adult due to your age
• are a carer who is open to Social Services in your own right
• or have an assessed need (including autism, ASD, dementia, or acquired brain injury)

You must also be in receipt of or be eligible for social care services in Powys. We currently support with issues  relating to an individual's care and support as per the Social Services and Wellbeing Act 2014.
