Please read the Job Description carefully before providing information in this Section of the Application Form. Give details of your previous employment or experience which you think would help you to do this job. (required): I have experience in personal care, I cared for Sam for 2 years he was 32 and had Duchenne muscular dystrophy (DMD) which is a severe, progressive genetic disorder causing muscle weakness and wasting. Because of his condition he had been in a wheelchair from a young age, so I would use a variety of hoists to safety transfer Sam from his bed to wheelchair, wheelchair to bathroom, toilet to shower chair, and he would love to scream ‘ahhhh’ just to wake me up on a early morning shift but also to give me himself something to laugh at (he would get me everytime), after he’d stopped laughing, I would assist Sam with personal care such as help with daily hygiene, showering, dressing, grooming, toileting, assisting with medication, as well as support with making him his favourite meals, and household chores. Once all personal care, medication and meals had been checked off our list, it was time for a bit of Fortnite, and Minecraft, and then we would pop out to the shop for some sweet treats before heading back home.


What qualities do you think are important when working as a personal assistant with a disabled person? (required): I think important qualities to have as a personal assistant with a disabled person is patience, understanding and reliability and to also have the willingness to learn more to help empower more independence.
How do you think you can contribute towards the needs and the independence of a disabled person? (required): By focusing on the individual’s ability’s and not the disabilitys. Learning, understanding and creating a safe and strong relationship that will hopefully build trust and confidence.


What is it about PA work which appeals to you? (required): I’ve always been interested in work that involves helping others and making a positive impact, I am motivated by being in a role where each day is different, I have empathy and a positive attitude, i am observant but with discretion. I feel personal care roles are where I thrive.


What are your hobbies/interests? (required): I have many hobbies and interests, I love to draw, I’m a gamer! And I love to sing, I feel anything I put my mind to I can do.


Would you consider a casual position if you are unsuccessful with this job? (required): Yes


Do you speak any other languages? if so what do you speak?  (required): I do not.


Do you drive? (required): No


Are you a vehicle owner?  (required): No


Do you smoke? (required): Yes

I will sadly not be able to provide a reference for my experience with Sam however if successful I will happily show you some pictures of us together, he did love a funny filter.
